Description:
Children with Medically Fragile Needs program assists families in keeping their child home instead of having to place them in a nursing home or hospital to receive needed care. A medically fragile child is defined as having a serious illness or condition, which is anticipated to last at least 12 or more months, who has medically intensive needs, and prolonged dependency on medical care and medical technology. The child may be medically stable at times but still may require skilled nursing care or specialized medical equipment and supplies to enhance/sustain their lives at home.
Medicaid Medically Fragile Waiver provides: Case management, in-home support, dietary supplements, counseling, equipment and supplies, environmental modifications, institutional respite.
