Northland PACE (Programs of All-Inclusive Care for the Elderly) serves adults 55 years of age and older living with and needing assistance with chronic medical conditions. Northland PACE is designed to keep adults that qualify for the program living at home safely and to preserve their ability to remain living at home as long as possible. Through a team approach we provide healthcare services at our Northland PACE day centers as well as healthcare professionals delivering services in the person's home. Participants of the PACE program and their family members are encouraged to play an active role in their plan of care, care decisions, and play a vital role to the Northland PACE healthcare team. Northland PACE provides services ranging from but not limited to: primary care providers, Nursing, Social Work, Dietitian, Physical Therapy, Occupational Therapy, Recreational Therapy, Home Care Coordination, Transportation, and more. The Northland PACE healthcare team provides all healthcare services needed by each participant that enrolls into the PACE program but also coordinates outside appointments with additional specialty care providers. Services are based on the needs of each individual participant of the program. As the needs of each participant changes their plan of care changes to meet those needs. There are no hidden costs, no co-payments or deductables, and no obligation to enroll upon inquiry or interest. Medicare and Medicaid are accepted and PACE services are paid for and provided by Northland PACE.
