Coal County Community Health Center with clinics located in Beulah, Center, Hazen, and Killdeer, ND provides comprehensive health services to its populations across the lifespan, including primary medical care services, diagnostic and screening services, outreach, health education, disease management, behavioral health, and substance use disorder services. In addition to your primary care visit, Coal Country Community Health Center also provides individualized comprehensive care coordination through Registered Nurse Care Coordinators who are available to work closely with you and your medical home team. Our Registered Nurse Care Coordinators will provide you and your family with individualized education, tools, and resources to manage your chronic disease within the safety of your home with a goal of optimal health and wellness.
