Community of Care is a nonprofit serving older adults and persons with disabilities, along with their family and friends, in rural Cass County. Information, assistance and referrals are available for a wide-variety of needs, including: financial assistance, equipment needs, housing, in-home care, support services, legal issues, medical needs, mental health/dementia, nutrition, tax assistance and transportation.
Insurance and Medicare questions, along with Medicare Part D Drug Plan enrollment are handled by staff trained as State Health Insurance Counselors (SHIC).
Care Coordination and Outreach are also available. The Volunteer Program includes these services provided by trained volunteers: medical and social transportation, minor home repair, yard work, assistance with shopping and errands, telephone reassurance and friendly visits. A Caregiver Support Group and BeFrienders are also available.
Community of Care recently added a Faith Community Nurse to the staff to serve older adults and others in need in northern Cass County. This holistic approach will focus on preventative care through visitation, counseling, referrals, education and support. A health team of local volunteers assist with this program.
